If you have a learning disability, you can ask to join our learning disability register. If you are on the learning disability register we can add information to your record, so we understand the best ways to talk to you and to help you get the health care you need. This is called reasonable adjustments.

If you are on the learning disability register and 14 years old or over, you can get a free Annual Health Check once a year. This is to help you stay well. You do not have to be ill to have a health check. You will be booked an appointment with one of our Nurses.
Please let us know if you have any questions or concerns about these checks as we may be able to help make you feel more comfortable.
Before your appointment you will be sent a questionnaire to fill out. If you prefer, you can ask a family, friend or carer to help you with this. There is a link to the questionnaire for your annual health check below, and also a checklist you might find helpful.

The Hertfordshire Community Learning Disability Nurses are part of the Hertfordshire County Council Adult Disability Services.
They can support any adult with a learning disability with getting any additional help they need to access health care. Information for people with learning disabilities, their family and carers, and professionals can be found on the links below.
The Purple Folder is a record of health information about you. We encourage all of our patients with learning disabilities to bring their Purple Folder along to all of their appointments. It helps doctors, nurses, pharmacists and other health professionals to give you the healthcare and treatment that is best for you.
You will need to fill it in, you can get family, a friend or carer to help you with this. Some people with a learning disability can get very ill if they get flu. The best way to avoid flu is to get a free flu jab. You can have the flu jab at the surgery. Everyone on the learning disability register is offered the flu vaccine every year.
